The Patient Protection and Affordable Care Act (Public Law 111-148) clearly represents a movement toward much-needed, comprehensive and meaningful reform for our nation’s healthcare system and it has affected nursing in different ways. In the provision of “Quality”, the Act emphasized on the importance of nursing care to patient outcomes, and placed a strong emphasis on reporting, both publicly and to the Secretary, of nurse staffing in long-term care settings and stated will ensure staffing accountability and enhance resident safety. This has not been achieved years later after the bill and is affecting the nursing workforce creating high turnovers, burnout, nurses changing careers and frustrations in the whole workforce. Currently, medical education institutions are unable to graduate the number of workers needed to guarantee broad access to medical care. Even with recent enrollment increases, according to statistics, demand will still outweigh the supply by 2025. Without a strong and growing workforce operating under better working conditions, the quality of patient care will not improve and the possibility of high-quality health care under the ACA will not be achieved. Most facilities today work short and under unconducive environment for example I work in a facility where shortage and bad environment has become part of the facility. As much as this affects patient care, it also affects nurses in giving quality care.

Comparative Effectiveness Research-Section 6301 which aimed at establishing a non-profit Patient-Centered Outcomes Research Institute to perform and synthesize research on comparative effectiveness. Its purpose being to help healthcare professionals in making informed health decisions and advance the quality and relevance of evidence concerning the manner in which health conditions can effectively be prevented, diagnosed, treated, monitored, and managed through research and evidence synthesis that considers variations in patient sub populations, and through the dissemination of research findings. This helps nurses in applying evidence-based practice according to researches done and ensures patient safety. Through reviewing of existing evidence based on benefits and harms and by comparing drugs, medical devices, tests, surgeries and ways to deliver care, it focuses on holistic nursing where it considers each patient as an individual with different needs and treatment to achieve desired results
